Welcome to Dog Gone Place Application Packet page.  Please click each link and complete each form separately.  After you have done so, you will have finished all the paperwork we require before bringing your dog in for his/her first visit with us.  All four (4) forms are Word documents so it will be easy to save them separately and email all of them to us as attachments.

Application Form will include all the standard information required to enter you into our customer database.

Medical Form can be handled in one of two ways depending on which way is most convenient for you:  a) print the form and have your veterinarian complete and sign it; or b) complete it yourself and bring it to us with a copy of an invoice or some other document of proof from your vet's office providing us with your dog's shot history.  If you have more than one dog, please complete a separate form for each dog.

Personality Profile will allow us to get to know your dog before meeting her/him including where he likes to be rubbed, what commands he knows, whether or not he's ever jumped over a fence, bitten someone or growled when toys were taken out of his/her mouth, etc.  As with the Medical Form, if you have more than one dog, please complete a separate profile for each dog.

Health and Temperament Form tells us all the information you are providing is true and you understand we are not liable for any problems arising with your dog as long as we have provided reasonable care and followed our standard precautionary measures.
