Welcome to Dog Gone Place's Application Packet page. Please highlight each link and complete each form separately. After you have done so, you will have finished all of the paperwork that we require before you can bring your dog in for his / her first visit with us. All four (4) forms are Word documents so it will be easy to save them separately and e-mail all of them to us as attachments in your e-mail.

Application Form will include all of the standard information required to set you up in our customer database.

Medical Form can be handled in one of two ways depending on which way is most convenient for you: a) print the form out and have your veterinarian complete and sign it; or b) complete it yourself and bring it to us with a copy of an invoice or some other document of proof from your vet's office that will provide us with your dog's shot history. If you have more than one dog, please complete a separate form for each dog.

Personality Profile will allow us to get to know your dog before we even meet him including: where he likes to be rubbed, what commands he knows, whether or not he's ever jumped over a fence, bitten someone or growled when toys are taken out of his mouth, etc. As with the Medical form, if you have more than one dog, please complete a separate profile for each dog.

Health & Temperament Form tells us that all of the information you are providing us is true and that you understand that we are not liable for any problems that might arise with your dog as long as we have provided reasonable care and followed our standard precautionary measures.
